Background Checks (Risk Management)
All adults affiliated with team management must be Risk Management certified. Submit your request below to access the coupon code for the application through your GotSoccer account.
Log into your Coach/Manager account on GotSoccer and select the Background Check tab to submit the application.
Injury Reporting
Must be submitted within 7 days of incident and must have occurred during a club related activity.

Registration Requirements
All documents below should be reviewed for accuracy before submitting to the club. DOB should be verified against an official document and the player's GotSoccer account. Documents with missing signatures or submitted with incorrect information will be returned for updates.

Uniform styles will change every two years. Next planned change will be Fall 2017.
